NOTICE TO PATIENTS REGARDING PAUL R. EHRMANN, D.O. POLICY FOR REGULAR COMPLETE PHYSICAL EXAMINATIONS
BEGIN JANUARY 1, 2007

Regardless of age, medical condition or lack of preventable insurance benefits, I strongly recommend that you have a complete physical examination with me on a yearly basis in order to establish baseline health information and for detection of any asymptomatic problems (you may have a problem that you are not aware of yet, that could be identified through preventative health examination). Please schedule this physical examination with our receptionist.

We also require that if you are a new patient that has signed up with me through your insurance company to be your primary care physician, that you be seen for a complete physical examination with 6 months of your sign up date in order to establish medical baseline information.
