No Show Appointment Policy In order to effectively monitor your medical care, we will schedule you for evaluations and reevaluations. We will always attempt to call you before your appointment time. If you are not home, we will leave a message about your upcoming appointment. If you know that you will be late to an appointment more than 15 minutes, please call our at 543 2000. If you know when you will not be able to keep your appointment, please call our office with at least 24 hour notice and use extension 230 to leave a message. In most circumstances, if you do not show for your appointment without prenotication as described above, you will be issued a postcard reminder about this missed appointment without charge to your account. . A missed second appointment, will result in up to a $50.00 dollar charge assessed against your account which will need to be paid before your next appointment with the Doctor. A missed third appointment without notification to our office will result in dismissal from the practice. You may also be charged up to $50.00 for missed complete physical exams, pap smears and procedures without 24 h office prenotification. Work Note Policy At the Family Health Care Center, we promise to try to get you in to see the doctor the same day. If a work-school note is needed, we will only be able to issue same on the first missed day and not after a patient has been out for more than one day, unless special circumstances exist as deteremined by the doctor. New patient sign up PCP policy When patients are signing up for some insurances , they list their Primary Care Physician (PCP) as their doctor at the start of the contract year. We strongly encourage new patients that have never seen their doctor that they have signed up for to make a "well" appointment within three months from sign up to establish establish baseline background medical information that is important when comparing in the future to "sick" visits. Please be advised, this well visit may or not be covered by your insurance.
